 Steps to Receive Private Insurance Reimbursement
Note: If you have an HMO (Health Maintenance Organization), or a PPO (Preferred Provider Organization), you must contact them prior to your purchase to determine if the SOFTEE® is a covered benefit under your health plan and if you are required to pre-authorize. If you fail to do so your insurance carrier may not pay.
When submitting a bill for medical reimbursement you must include:
Note: If you have BlueCross/BlueShield Insurance and would like to receive a BC/BS medical reimbursement form, you must put a check in the provided box at the time you place your order.

A pre-authorization number if applicable |

2. |
The original invoice or receipt |
|
3. |
A completed and signed standard medical reimbursement form (#1500 form) |

4. |
A prescription from your physician
|

Note: If you have Blue Cross/Blue Shield Insurance, you must check the box at Checkout. You will submit your claim for
reimbursement on a red & white original CMS 1500 form that we provide. |

Submit the original paperwork to your insurance company and always keep a copy of each item for your records

About Private Insurance, HMO, PPO and Managed Care
The SOFTEE® is covered for medical reimbursement by many private insurance companies and Health Maintenance Organizations. Contact the benefits department of your carrier to clarify coverage, how much they will reimburse, and if you need prior authorization.
The industry billing code number for the SOFTEE® is the same as for Medicare, #L8015. The medical item description is also the same "External breast prosthesis garment, with mastectomy form - post mastectomy". At your request, we will include a copy of this standard insurance claim
form (#1500) inside your SOFTEE® shipment. You can also download the form using the link above.
